หนังสือยินยอมให้เก็บรวบรวม ใช้ หรือเปิดเผยข้อมูลส่วนบุคคล

สำหรับผู้ที่สนใจศึกษาต่อ หรือสนใจข้อมูลของมหาวิทยาลัย

ด้วยพระราชบัญญัติคุ้มครองข้อมูลส่วนบุคคล พ.ศ.2562 เป็นกฎหมายที่มีเจตนารมณ์เพื่อคุ้มครองสิทธิความเป็นส่วนตัวของข้อมูลส่วนบุคคลโดยกำหนดให้การเก็บรวบรวม ใช้ หรือเปิดเผยข้อมูลส่วนบุคคลจะต้องขอความยินยอมพร้อมทั้งแจ้งวัตถุประสงค์ให้เจ้าของข้อมูลส่วนบุคคลทราบโดยชัดแจ้งเว้นแต่เป็นการดำเนินการที่กฎหมายอนุญาตให้ใช้ข้อมูลส่วนบุคคลเพื่อประมวลผลโดยไม่ต้องได้รับความยินยอมจากเจ้าของข้อมูล

โดยหนังสือฉบับนี้ มหาวิทยาลัยเกริก หรือ “มหาวิทยาลัย” ถือเป็นผู้ควบคุมข้อมูลที่มีหน้าที่ตามกฎหมายในการคุ้มครองข้อมูลส่วนบุคคลที่อยู่ในการครอบครองหรือควบคุมของมหาวิทยาลัย มีความประสงค์ขอความยินยอมในการเก็บรวบรวม ใช้ หรือเปิดเผยข้อมูลส่วนบุคคลที่เป็นทั้งข้อมูลทั่วไปและข้อมูลอ่อนไหวสำหรับสำหรับผู้ที่สนใจศึกษาต่อ หรือสนใจข้อมูลของมหาวิทยาลัยเพื่อติดต่อสื่อสารให้ข้อมูลหรือคำแนะนำ หรือทำการตลาด หรือจัดกิจกรรมประชาสัมพันธ์ และให้คำแนะนำด้านการเรียน ตามวัตถุประสงค์ที่ระบุไว้ในหนังสือแจ้งการประมวลผลข้อมูลส่วนบุคคล (Privacy Notice) สำหรับผู้ที่สนใจศึกษาต่อ

สำหรับผู้ที่สนใจศึกษาต่อ หรือสนใจข้อมูลของมหาวิทยาลัยหรือ “ข้าพเจ้า” ขอให้ความยินยอมกับมหาวิทยาลัยในการเก็บรวบรวม ใช้ หรือเปิดเผยข้อมูลส่วนบุคคล ดังนี้

(1) ข้าพเจ้ายินยอมให้มหาวิทยาลัยเก็บรวบรวม ใช้ หรือเปิดเผยข้อมูลส่วนบุคคลของข้าพเจ้าที่เป็นทั้งข้อมูลทั่วไปและข้อมูลอ่อนไหว เพื่อให้มหาวิทยาลัยนำไปใช้ตามวัตถุประสงค์ที่ระบุไว้ใน หนังสือแจ้งการประมวลผลข้อมูลส่วนบุคคล (Privacy Notice) สำหรับผู้ที่สนใจศึกษาต่อ

(2) ข้าพเจ้ายินยอมให้มหาวิทยาลัยเก็บรวบรวมข้อมูลส่วนบุคคลของข้าพเจ้าจากแหล่งที่มาตามที่ระบุไว้ใน หนังสือแจ้งการประมวลผลข้อมูลส่วนบุคคล (Privacy Notice) สำหรับผู้ที่สนใจศึกษาต่อ

(3) ข้าพเจ้ายินยอมให้มหาวิทยาลัยเก็บรักษาข้อมูลส่วนบุคคลของข้าพเจ้า เท่าที่ระบุไว้ใน หนังสือแจ้งการประมวลผลข้อมูลส่วนบุคคล (Privacy Notice) สำหรับผู้ที่สนใจศึกษาต่อ เว้นแต่กรณีจำเป็นต้องเก็บรักษาข้อมูลต่อไปตามที่กฎหมายกำหนด

(4) ข้าพเจ้ายินยอมให้มหาวิทยาลัยและผู้ปฏิบัติงานอื่นของมหาวิทยาลัยนำข้อมูลส่วนบุคคลของข้าพเจ้าที่ได้เก็บรวบรวม ใช้ เพื่อนำไปดำเนินการจัดการศึกษาหรือกิจการอื่นใดที่เกี่ยวข้องกับการดำเนินงานของมหาวิทยาลัยตามที่ระบุไว้ใน หนังสือแจ้งการประมวลผลข้อมูลส่วนบุคคล (Privacy Notice) สำหรับผู้ที่สนใจศึกษาต่อ

(5) ข้าพเจ้ายินยอมให้มหาวิทยาลัยเปิดเผยข้อมูลส่วนบุคคลของข้าพเจ้าให้แก่หน่วยงานของรัฐและหน่วยงานภายนอก ตามที่ระบุไว้ใน หนังสือแจ้งการประมวลผลข้อมูลส่วนบุคคล (Privacy Notice) สำหรับผู้ที่สนใจศึกษาต่อ

(6) ในกรณีที่มหาวิทยาลัยมีความจำเป็นต้องส่งหรือโอนข้อมูลส่วนบุคคลไปยังต่างประเทศ (ถ้ามี) มหาวิทยาลัยจะขอความยินยอมจากข้าพเจ้าอีกครั้งตามที่ระบุไว้ใน หนังสือแจ้งการประมวลผลข้อมูลส่วนบุคคล (Privacy Notice) สำหรับผู้ที่สนใจศึกษาต่อ

(7) ข้าพเจ้าทราบว่าข้าพเจ้ามีสิทธิตามที่ระบุไว้ใน หนังสือแจ้งการประมวลผลข้อมูลส่วนบุคคล (Privacy Notice) สำหรับผู้ที่สนใจศึกษาต่อ และมีสิทธิที่จะเพิกถอนความยินยอมทั้งหมดหรือบางส่วนเมื่อใดก็ได้โดยแจ้งให้มหาวิทยาลัยทราบและมหาวิทยาลัยอาจขอทราบเหตุผลแห่งการนั้นได้ แต่การเพิกถอนความยินยอมนี้จะไม่ส่งผลกระทบต่อความยินยอมที่ข้าพเจ้าได้ให้ไปก่อนหน้านี้ ในกรณีที่การเพิกถอนความยินยอมกระทบต่อสิทธิหรือหน้าที่ใดๆ ของข้าพเจ้า หรือทำให้ข้าพเจ้าไม่ได้รับสิทธิประโยชน์หรือการบริการจากมหาวิทยาลัยอีกต่อไป ข้าพเจ้ายอมรับผลกระทบที่เกิดขึ้นจากการนั้นได้

ข้าพเจ้าได้อ่านและเข้าใจในหนังสือแจ้งการประมวลผลข้อมูลส่วนบุคคล (Privacy Notice) สำหรับผู้ที่สนใจศึกษาต่อ หรือสนใจข้อมูลของมหาวิทยาลัยโดยตลอดแล้วและข้าพเจ้าขอรับรองว่าข้อมูลที่ได้ให้ไว้กับมหาวิทยาลัยถูกต้องตามความเป็นจริงทุกประการ หากข้อมูลดังกล่าวไม่ถูกต้องตามความเป็นจริงท่านตกลงยินยอมให้มหาวิทยาลัยดำเนินการตามกฎหมายหรือชดใช้ความเสียหายที่เกิดขึ้น แล้วแต่กรณี
